Etrieve Forms
You must be a Current Student to use the Online Form option using the single sign-on through Self-Service. In order to access and utilize the Online Form option through the single sign-on feature in Self-Service, you need to be an active and enrolled student at the present time. This requirement ensures that only current students have the privilege to use the Online Form feature, which may involve submitting forms or accessing specific resources related to their student status.
How to Access Online Forms
- Click on the 'View Online Forms' button found above. You may be asked to provide your CEI login to authenticate.
- Find the available fillable forms under the 'Forms' tab highlighted below:
- Once the desired form is completed, click the "Submit" button on the bottom left below your form. That form is complete.
